The following are quick links to forms for ICS use only and commonly used HR forms. If the HR form you are looking for is not here search the Macquarie personnel forms at http://www.pers.mq.edu.au/Forms/

For Advertising

Request to Advertise
Submit to ICS HR Manager with draft advertising copy and ICS MORS User Access Form (below)

Form to Establish Selection Panel and Access Applications
All members of the selection panel to sign - Dean to sign off (as approval of panel)

report on verbal reference check
use to record information obtained from a verbal reference check (usually for General Staff positions)

proforma request for written referee report

email or write to request referee report on applicants for academic positions

For Appointing and Reappointing
Academic Staff Selection Report
If position was advertised use this form to report on outcome of interviews. Hiring manager to complete - include list of all candidates, shortlisted, non-non shortlisted and recommendation(s)

General Staff Selection Report

If position was advertised use this form to report on outcome of interviews. Hiring manager to complete - include list of all candidates, shortlisted, non-non shortlisted and recommendation(s)

PLUS FOR ALL RECRUITMENT WHETHER ADVERTISED OR NOT

For first time appointments submit relevant form below with resume, copies of qualifications, references and visa (if not an Australian Citizen). First time casual appointments also require Tax File Declaration form and Banking Authority
Academic Staff (fixed-term & continuing)
General Staff (fixed-term & continuing)
Academic staff (Casual)
General Staff (casual)

For ending an appointment
Expiry of fixed-term appointment
allows HR to release final pays including accrued annual leave

For taking leave
leave (full-time staff)
use for annual leave, personal leave including sick leave, family emergency leave, bereavement leave form moving house (include a change personal details form), long service, leave without pay.

Statutory Declaration
use for periods of family leave in excess of 3 days where a medical certificate is not available

Timesheets
Overtime claim- general staff
Use this form to claim additional hours if you are continuing or fixed-term ( full time or part-time).

Timesheet - casual general
Use this form if you are on a Casual General Staff member


Timesheet - casual academic
Use this form if you are a Casual
Academic Staff Member (i.e. you do demonstrating, tutoring, marking, lecturing or other hourly academic work)

variable hours roster
for general staff who wish to work 70hrs pf over less then 10 days
